Job Description:
GENERAL STATEMENT OF DUTIES:
As an employee of the facility Level of Care Registry, a Licensed
Vocational Nurse (LVN) may be requested to work on any shift, any
day of the week and on any residence at the Developmental Center.
Under the direction of supervisory staff, he/she provides direct
care, treatment and developmental services for the clients where
assigned. The services provided fall within the scope of the job as
specified by the California State Personnel Board .
SUPERVISION RECEIVED:
Works under the supervision of the Registry Coordinator,
Supervising Registered Nurse (SRN), Unit Supervisor (US), Shift
manager (SRPT), and/or Acting Nursing Person in Charge (NPIC) of
the residence.
ESSENTIAL FUNCTIONS : Must possess and maintain sufficient
strength, agility, endurance and sensory ability to perform the
duties contained in this duty statement. Must be able to lift 50
pounds without assistance; must be able to participate in client
containment, intervention or restraint. Must have the ability to
work extended hours and various shifts; must float to all work
areas within the facility if called upon to do so.
TYPICAL WORKING CONDITIONS:
On-going interaction with clients, personnel and the general
public. Potential exposure to communicable diseases. Blood borne
pathogens and other conditions common to a clinical nursing
environment.
DUTIES AND RESPONSIBILTIES:
5% - Performs nursing procedures, such as administering medications
and treatments including oral medication, hypodermic injections,
start and monitor IV's (with appropriate training/certification),
ventilator care, urinary catheterization, enemas, and taking and
recording temperature, pulse, blood pressure, respiration, and
first aid as authorized within the scope of the Licensed Vocational
Nurse.
5% - Participates as a member of the Interdisciplinary Team in all
aspects of the development and implementation of the Individual
Program Plan.
5% - Observes and documents clients' physical condition and
behavior and reports significant changes to appropriate staff.
5% - Provides one-to-one supervision/direct observation of clients
on special precautions, I.E. elopement risks, suicide risks and
medical condition and risk of serious self-injurious behavior to
client.
5% - Maintains a safe, sanitary, and therapeutic environment which
promotes respect and dignity and protects the privacy, rights,
confidentiality and physical/emotional well-being of the
clients.
5% - Responds to emergencies that involve use of medical/behavioral
intervention techniques.
5% - Escorts clients on the facility grounds and to/from the
community.
5% - Works extended hours and/or varying shifts.
5% - Prepares physical assessments, and develops health care
objectives as indicated.
5% - Works at various sites as needed to meet staffing
requirements.
5% - Assists clients in all activities of daily living, such as
bathing, dressing, grooming, dining and leisure activities. These
duties also include lifting, positioning and client mobility.
5% - Assists nursing staff, in collaboration with the physician, in
determining what, if any, habilitative training should be continued
while patient remains on acute unit.
5% - Attends and participates in training as required.
TYPICAL TASKS:
15% - Primary care giver for assigned clients.
10% - Remains flexible in staffing patterns and resolution of
staffing conflicts, including rotation/floating to units as
assigned.
10% - Doing weekly/monthly charting and Independent Living
Assessment for Interdisciplinary Team. Attend training classes.
