Ensure continuity of care by providing direct clinical nursing and
supervising CNAs, LVNs and support staff Assess, plan, implement
and evaluate individualized nursing care for residents in the
skilled nursing setting Communicate promptly and clearly with
physicians and the interdisciplinary health team about changes in
resident condition Respond quickly and accurately to changes in
clinical condition and to treatment responses Lead and mentor staff
as the charge nurse to promote high-quality, compassionate care
Participate actively in Quality Improvement activities to enhance
resident outcomes Maintain accurate documentation in the facility
record and support Electronic Medical Records workflows Interface
professionally and respectfully with residents, families, staff and
regulatory representatives Qualifications Current California RN
license (required) Minimum two (2) years prior experience in a
long-term care/skilled nursing facility Current BLS certification
(required) Working knowledge of Title 22, OBRA and long-term care
regulations Supervisory or charge nurse experience preferred
Ability to pass the nursing medical test prior to assignment
Proficient English communication skills, both verbal and written
Basic computer skills; EMR experience is a plus Education Active
California Registered Nurse (RN) license (required) Current BLS
certification Knowledge & Skills Patient assessment, care planning,
implementation and evaluation Supervising CNAs, LVNs, RNs and
support staff with strong leadership Clear communication with
physicians and multidisciplinary teams Quality Improvement
participation and regulatory compliance Familiarity with Electronic
Medical Records Practical knowledge of Title 22 and OBRA
