Responsibilities: - Provide comprehensive primary care services to patients of all ages, including preventive care, acute care, and chronic disease management. - Conduct thorough medical assessments, diagnose illnesses, and develop personalized treatment plans. - Collaborate with interdisciplinary healthcare teams to ensure coordinated and holistic patient care. - Educate patients about health maintenance, disease prevention, and lifestyle modifications. - Utilize electronic medical records (EMR) to accurately document patient encounters and maintain up-to-date medical records. - Participate in quality improvement initiatives and adhere to evidence-based medical guidelines. Preferred Qualifications: - Experience working in a community health center or primary care setting. - Bilingual proficiency in English and Spanish. - Demonstrated commitment to serving underserved populations.
